To ensure that your visit to our practice is efficient, that you receive timely service from our staff, and that our physicians can provide you with the best possible care, we ask that you provide us with the following when you come for your visit:
- Insurance card(s).
- Referral forms from your primary care provider if required by your insurance plan.
- Pertinent x-rays, lab results, and previous records that you may have relating to the condition for which you are seeing our physicians. Please bring a CD with any recent imaging procedures to the visit.
- Your personal medical history, including a list of your medications and allergies.
